Examples of eligible professionals include primary care providers, nurses, therapists, counselors, psychiatrists, and others with relevant licensure.
HUD guidelines specify that the health professional must be licensed, and typically, they should be licensed in the same state as the patient for whom they are providing care. According to HUD, healthcare professionals should base their documentation on personal knowledge of their patient/client—this includes knowledge used in diagnosing, advising, counseling, treating, or providing healthcare or disability-related services.
