How do I apply for an assistance animal?

If you are a new resident and you are applying for an assistance animal, please complete the following steps.

  1. When you are getting started with your animal profile, please indicate whether your animal is a pet, service animal, or ESA.
  2. If you are not logging in for the first time, you may create a new animal on your profile. Please select your animal type on the dropdown. 
  3. After you have completed your animal profile, please upload all necessary information to complete your assistance animal application.
    • Assistance animal documentation
         
    • Health professional first/last, email, phone or fax number

    • Confirm the method through which the documentation was obtained

    • Authorize OurPetPolicy to reach out to the provider for verification
    • Review the Privacy Policy for assistance animals
